Using the collective impact theory, we are working together to develop a common agenda; establish shared measurements; foster mutually reinforcing activities; encourage continued communication; and build strong community support.

By using the principles of collective impact, Achieve Healthy EscaRosa brings together stakeholders and policy makers that have not traditionally been in the health improvement discussions. Focusing on health equity, our team works to lift up residents who, historically, have suffered from health inequities.
2021 Healthy EscaRosa Founders
The Healthy EscaRosa Founders is a prestigious group of leaders dedicated to tackling the social determinants of health in our community. They are responsible for the funding of AHER's first project, the 2021 CHNA Report.
We would like to recognize these founders for their contributions to this community initiative.
