AHER launches newest plan to improve community health
AHER is proud to announce the launch of the newest 2026- 2028 Escambia and Santa Rosa County Community Health Improvement Plan.
Improving health outcomes for Escambia and Santa Rosa County resident's through the alignment and uniting of local resources is an integral part of Achieve Health EscaRosa (AHER). Every 3 years, AHER conducts a Community Health Needs Assessment (CHNA) in collaboration with local sponsors and releases the report outlining the community's health needs and driving meaningful action.
After completion and review of the CHNA, health care organizations, educational institutions, non-profits, civic and community partners, and health departments meet to identify top health priorities and create goals to improve outcomes related to the significant health needs. A 3-year blueprint known as the Community Health Improvement Plan (CHIP) is created to provide a practical, descriptive framework for community-decision making and activity prioritization.
